the 200 amp has a lot of mass, so belts could slip if loose. Your description of it only happening when the engine is revving up under heavy load though would make me suspect it doesn't have anything to do with the belts, as even if slipping the alternator shouldn't be dropping in speed when...
How old are your batteries and have you had them tested?
I noticed a few issues with the voltage not staying steady when I had a bad battery once upon a time.
The 90 amp rating is likely just the startup surge; actual running amps would likely be a lot less than that. It would have to have something like 6 gauge cable in order to have a continuous 90 amp draw.

Just for an update, I've now talked to several other people with sheared off exhaust stud and sheared off bolts in the front, the same thing as mine did. They have gone to the newer style bracket with no difficulty and are very happy with the upgrade.
my truck has started in frozen weather with at least 4 bad glow plugs, maybe more. I've replaced 3 of them so far.
It does put out quite a bit of smoke for the first 10-20 seconds or so, but runs perfectly after that.
I'd say for only 2 stck glow plugs it isn't worth the time/money to tear it...
The idler pulleys use a replaceable bearing in them that isn't that hard to change out. If nothing else keep a few of them on hand.

Alternator bracket had the rear brace stud on the exhaust sheared off, and that put extra stress on the front bolts, two of which sheared off and the third loosened. Ended up replacing the rear bracket with the newer one.

You could probably make them fit, but they are too soft for the added weight of the M1165 over the basic M998, and would be more compressed, losing some height. Those springs are designed for a truck weighing no more than 7700 lbs fully loaded, and empty your M1165 is edging up on that.
