Kind of a tighten, spin a few times and back off a quarter turn, type deal, before replacing the cotter pin, or at least that is what I do.
By no means a master mechanic, just what I do since burning out bearings on my car when I was young for having them too tight.
I believe they are also called dog bones. and yes you need to replace the entire "dog bone".
There is one thread showing a patch job to get it home by welding a cross piece to hold the hoop part of the dog bone onto the stud.

I've heard how much effort it is to get run flats into the tire, I can only imagine how fun it is to remove them.
I think removing them would be the last option, unless you truly do not want them in the tire.

That supports the TM which shows dex for 75+ temps and the oil solution for 100+ temps. That was one reason for my interest in this thread. Is the synthetic best of both worlds or just an expensive version of dex.
I will be driving in 100+ temps but unlikely to be fully loaded or extreme off...
