A couple of SF-97 questions

Hello everyone. I am getting ready to fill out my first SF-97 and had a couple of questions. First it is a 1970 M35a2 made by Kaiser Jeep. Which is better to list it as? Kaiser or Jeep. Less hassle? Second, Do you use the long number on the dash plate or the number on the frame? Luckily mine match.
