Question on Make and Model

I am getting ready to do my SF-97 on my M1008 and wanted to know what to put down. The Data plate in the door jam says make and model then that is followed by CD-30903. Now is CD the make and 30903 the model? Do I put this down instead of Chevy as the make and I don't even know what model Chevy it would be?

Model: M1008

I usually put make: Chevrolet and Model: D30. Our state has no idea what a M1008 is.
