Confused with filling SF-97 form, Please help

When going to fill the form I got confused as to what should go where.
Things i couldn't figure out are:
The manufacturer name
The year it was made
VIN No.
weight VS GVWR (I can see "few" weights and no GVWR)

I'm posting pics of all the markings
(the cover I got doesn't belong to this trailer so ill have to find one, if anyone have one please let me know)

01084 is the cage code for Silver Eagle, so I'd say Silver Eagle is the manufacturer.

GVWR is 3400 lbs

GVWR = Curb Weight + Payload

In this case, 3400 = 1460 + 1940.

I'm not familiar enough with the trailers to help you with the VIN or year of manufacture.

Use the mfr serial number for the vin.

Weight=Gross weight 3400lbs
Usually when you title it in your state they will want the trailer length too. Just measure from the hitch eye to the rear. Hope this helps you.
