Data plate for ether starting kit

Does anyone know where I can get a replacement data plate for the ether starting kit? I checked on TNJ Murry's website and they don't seem to have it. I checked the MV Data Plates website and they don't seem to have it. I also checked the Saturn Surplus site and they don't have it either. A Google search for the part number written on the back of the data plate doesn't turn up anything. I searched through a dozen pages on eBay and could not find one there either. The number on the back is 40-3890007. Any advice you have is greatly appreciated.

If you cant find one then you can just send it or a scan of it to Greg at dataplates.net in fact he may already have that one. He is the go to guy for plates as of the last year or 2. Fast and affordable.

Thanks, Aaron. I sent dataplates.net an email with a photo attached on the 18th but have not heard back from them yet. I'll give them a call in a couple days.
