On mine, there are two parts to this. The spacer, and the actual boot. Info below.

5715542 SPACER, HALFSHAFT BOOT (TULIP) [12.1 GVW]
57K3515 KIT, HALFSHAFT BOOT (12.1 GVW) --- Out of stock everywhere ---

Look at the picture here:

See also thread "CV BOOT INSTALLATION", post #49, for an alternative to the 57K3515. The 686-2326 boot fits a lot of vehicles; my local NAPA store shows it in stock, $23.99 each. The 686-23252 boot fits a number of Honda vehicles but does not appear to be commonly stocked at stores; it is available by next day shipping to home or store.

if you use one of these, how about report back on how it went? I still have not had a need to replace the boots on a 12K axle...

Just a heads up: When I replaced my boots, I observed two issues. 1. I could not pull the shaft out of the inner joint. I had to slide the boot off over the outer end of the shaft. Application of BFI did not work. That snap ring would not budge. (Scott from Southern Axle Exchange confirmed that that can be an issue). Same for the installation of the new boot. Slide it on from the outer end.
2. The Tulip spacer was so swollen up (apparently from the oil / grease) that it was hard to reinstall. I wound up replacing them.
