if i had to say, that is sort of guess, i would say that most gear tooth breakage was directly related to some other failure where some small piece went between the gear teeth. 5th has the lowest torque and is possibly the best supported of the gears so i would expect it to fail least otherwise...
rebuilt by the mil means nothing. well maybe it means something, but that something is that it may not be as good as a used trans, i have seen some of the crap used worn out parts they put back in and you would not want them in your trans.

i am not sure what you gain with disk brakes on a truck like this. the shortcomings in braking are not so much the drum brakes, but the booster and the low pressure it generates. my re-engineered brake systems have all had stock brakes at the wheels, but you can lock them easily with one finger...
2100 rpm with 395s and .69 od is 62 mph. it would be nice but the multifuel will never do this. otherwise the .69 offers just a very little bit more mph at any given rpm over 395s, this with 14.5r20

it will kill the performance. for the same or even less money you can add .69 overdrive.
certainly less work than spreading rear axles.
mph = ( rpm * radius) / ( 168 * ratio)

yeah ron, good question! well we all know that it won't work, but 2000 rpm would be 91 mph just in case you wonder. i spend nearly 4 years and untold thousands of $$$ developing this and with the help of some good friends too and this ratio is all that the multifuel can handle. those who have it...
if it is modified, it is no longer a historic vehicle. sorry, but it just isn't, it now is a custom truck and you should get the appropriat insurance. if you don't and have a claim, don't expect them to stand behind it. many people are 'getting away with it', but sooner or later they will regret...
5th is not .85, never was and never will be. fuel economy will not be gained by any sort of overdrive, at least not in any real way. some small gain is had with my od gear set, but only if you keep mph at 55 or less.

you really should read the manuals. if you don't get it, read them again. there will be lots of other stuff you will learn, like where the brake fluid goes;)

there are 2 'o' rings that you must replace. they should come with a new head. be very careful not to overtighten the fuel lines. i would have to look up the torque but i would guess maybe 20 ft lbs. too much can cause damage.

running hot can often be directly linked to leaking head gaskets, not blown, but just leaking enough to heat the coolant and add some pressure to the radiator. there is a chemical test where you pull some of the air from the radiator to see if there is combustion gasses in it. i have seen this...
