Just recently checked spindle nut on my 85 998 1038 . It has the old 4 slot spindle nut. Tab is secure and holds just fine. My question is should I upgrade to 8 slot nut ? If so can I just replace the nut itself or do I need to replace washers and spindle nut washers as well ?

I would, there is a reason there is an "upgrade" for that part, I always replace the "washer, key" any time the nut is removed, just too cheap not to.
I just looked and AMG has both the nuts and the tabs listed, under $20 for the pair plus shipping of course, fleabay even cheaper

My thought on this is- If you're going to upgrade, upgrade to the absolute safest option. None of the AMG stuff is very good or reusable. Buy this kit from Mod Mafia or one of the other vendors or don't touch it at all.

Yes, it's stupidly expensive but it's the most secure locking method and it's totally reusable. You'll never buy it again unless you booger it up during removal or installation.

You are right that is stupidly expensive,

Isn't the issue from improper installation? Or reuse of old parts. Has anything fell apart that was assembled at the factory?

My understanding is- "Yes."

The Army had a massive program of rechecking every HMMWV and marking them on the body that they were "checked." This was after factory assembly.
The first tabbed washers only engaged a single finger. The second style engages 2 fingers. The 3rd style is still not reusable.

I really try not to buy too much into aftermarket hype but this is one case where I feel the aftermarket product is markedly superior. You won't catch me putting those "heat sink" portal hub covers on my truck, for example.
