You have two options as I see it. Either use the "Lift" shackle points to the front of the fifth wheel and some kind of clamp/binder or you can use the rear "Tie down" points and binders pulling against the bottom of the box which mates up to the ramps...

Should we also assume that the trailer is a "hi-cube"? - which is pretty much all the modern ones, in that it max's out at 13'6" on a very low fifthwheel.
