In the PCB there is something called a reverse polarity relay that will inhibit starting to protect the truck if the batteries get installed backwards so it is possible that you have a bad PCB but with their high cost I'd make sure it was the problem before changing it out.

If you jumped it from the STE/ICE connector and it worked you just verified the starter and solenoid are good. The start switches do fail occasionally. I'd also look for a broken wire or a separated connector.

First question, is it the original alternator and how many miles are on the truck? Sounds like the brushes in the regulator are worn out and not making good contact at RPM. The brush block is available from Southern Automotive as a separate part here...
