I don't have an answer to your question, but i have crawled around under a few of these and I'd suggest you consider fitting a transfer case PTO instead of the transmission PTO.
You will have more space under the bed than if you try to fit a front and/or rear facing Transmission PTO.

It's a long shot, but you may find a problem in the knuckle area: stub axle not floating properly, worn bushings, worn out cross.
