Hmm, definitely sounds like some sort of trouble code. I searched around a little on the net but couldn't find a troubleshooting guide for the smart start box. It sounds like you have done all of the usual stuff like the ground harness, etc already. Maybe a bad box? Did you get it from Kascar...
Thanks George. I have looked all along that lip. I'm guessing it is hidden under a layer of paint. The previous owner had the tub media blasted before it was repainted, so I guess it's possible the number was blasted off in the process.

I have always been good about wearing a shield when I am welding. I usually use an auto-darkening unit, which works well in most situations, unless I'm in a tight spot. I don't usually have ear protection on unless I am going to be grinding for an extended period, but I may change my habits...
