Don't limit yourself to just deuces either. Be sure to take a look at LMTVs, 939series 5tons, 809series 5tons and maybe even MTVRs.
They all make fine toys.
I had a 2004 Ram 2500 with 5.9 Cummins and auto transmission. 3.73 gears. Stock tires. 21mpg generally and never less than 16mpg. I never really towed heavy with it.
I currently have a 2008 ram 3500 with 6.7 Cummins and G56 6spd manual transmission. 4.10 gears. 34" tires and a couple inches...
M1009 CUCV will get 20ish in stock form empty.
I doubt you're going to do much better than that no matter what you do to the M1008.
Taller tires = poor aero. The taller, the more pronounced.
Taller gears = better mpg, to a point. That point is the point that the engine no longer makes...

I come to the conclusion that EH likes 33's and is tweaking the other variables to optimize for them. (This is not a criticism).
Personally, I'd be reluctant to give up the 4.56s. Also, I'd rather spend the extra $ on 37" tires than spend the $ on a gear ratio swap. But...
You made the right decision. An M116A3 is a much better choice for a trailer that will be pulled with a pickup truck.
I've had both. I like the M200A1s but they require an airbrake truck to activate the brakes.
